After you have your Life Care Planning forms completed in Arizona It may be required to be notarized and executed in front of witness's.

Reschedule and Cancellation Policy for less than 24 hour notice:
- Reschedule: $25.00 Reschedule Fee
- Cancelled without rescheduling: 50% cancellation fee deducted from refund.
